ICU舒适护理模式在术后气管插管清醒患者中的应用研究

摘要目的探讨ICU实施舒适护理模式对外科术后气管插管清醒患者的影响。方法选择2015年6月～2016年7月航天中心医院外科四级手术后带气管插管回ICU的264例清醒患者为研究对象，按照入院时间不同分为对照组（137例）和干预组（127例）。对照组给予常规护理，干预组给予舒适护理。比较护理前后两组患者的舒适度、对护理服务的满意度、遵医行为及术前术后焦虑评分。结果两组患者均存在中等程度的不舒适（依据VAS评分标准）。干预组患者的护理满意度、舒适度、遵医行为均高于对照组，差异均有统计学意义（P ＜ 0.05）。术前两组患者焦虑评分比较，差异无统计学意义（P ＞ 0.05），术后干预组焦虑评分明显低于术前，差异有统计学意义（P ＜ 0.05），但对照组术前术后焦虑评分比较，差异无统计学意义（P ＞ 0.05）。结论舒适护理措施可提高ICU术后气管插管清醒患者的满意度和舒适度，提高其遵医行为，降低焦虑程度，帮助患者顺利度过气管插管机械通气期，促进康复，值得临床推广应用。

Abstract：Objective To explore the influence of comfort care model in the postoperative conscious patients with tracheal intubation in ICU. Methods From June 2015 to July 2016, 264 cases of conscious patients with tracheal intubation after Level four surgery in Aerospace Center Hospital were selected as research objects and divided into the control group (137 cases) and the intervention group (127 cases) based on the admission time. The control group was given routine nursing care and the intervention group was given comfort care model. The comfort level, nursing satisfaction, compliance behavier and the score of anxiety were compared between the two groups. Results Patients in the two groups had moderate degree uncomfortable (according to the scores VAS scale). The nursing satisfaction, comfort level, compliance behavior in the intervention group were higher than those of control group, with statistically significant differences (P ＜ 0.05). Before operalion, there was no statistically significant difference in the scores of anxiety in the two groups (P ＞ 0.05). After operalion, the score of anxiety in the intervention group was lower than that of before intervention, with statistically significant difference(P ＜ 0.05), but it was higher in the control group, with no significant difference compared with that of before intervention (P ＞ 0.05). Conclusion Comfort care model can improve comfort level and nursing satisfaction in postoperative conscious patients with tracheal intubation in ICU, promote the compliance behavior, relieve the anxiety, assist the patients pass through the mechanical ventilation period, promote recovery. It is worthy of clinical promotion and application.
