综合护理干预在断指再植术患者中的应用效果

摘要目的探究综合护理干预在断指再植术患者中的应用效果。方法选取2015年7月～2017年11月合肥市第二人民医院显微骨科收治的62例接受断指再植术患者，按随机数字表法分为对照组与观察组，每组各31例。对照组患者采取常规护理措施，观察组患者在常规护理措施基础上接受血管危象护理、综合心理护理、精细作业康复训练及中医护理干预。两组患者出院时分别统计断指成活率，血管危象发生情况与护理满意度，并在护理干预前与术后3个月通过手指总主动活动度评价法（TAM）、普渡钉板测试（PPT）及明尼苏达手灵巧度评定法（MMDT）评估患指功能恢复情况。结果观察组断指再植成活率高于对照组，血管危象发生率低于对照组，术后3个月TAM、PPT与MMDT结果优于对照组，护理满意率高于对照组，差异有统计学意义（P < 0.05）。结论优质综合护理干预有效促进断指再植术后患指成活与功能恢复，具有较高的应用价值。

Abstract：Objective To investigate the application effect of comprehensive nursing intervention on replantation of amputated finger. Methods From July 2015 and December 2017, in Department of Microscopic Orthopaedic, the Second People′s Hospital of Hefei City, 62 patients with replantation of amputated finger were selected. According to random number table method, they were divided into observation group and control group, with 31 cases in each group. The control group was given routine nursing care, the observation group was given vascular crisis nursing, comprehensive psychological nursing, delicate occupational rehabilitation training and traditional Chinese medicine nursing intervention based on routine nursing care. The survival rate and vascular crisis occurrence rate of severed finger, nursing service satisfaction in two groups were compared after patients leaving hospital, before nursing intervention and 3 months after surgery, severed finger function was assessed by total active movement (TAM), Purdue pegboard test (PPT) and Minnesota manual dexterity test (MMDT). Results The survival rate of the replantation severed finger in the observation group was higher than control group, the incidence of vascular crisis was lower than control group, the results of TAM, PPT, and MMDT were better than control group 3 months after surgery, the rate of nursing satisfaction was higher than control group, the differences were statistically significant (P < 0.05). Conclusion The comprehensive nursing intervention can effectively improve severed finger survival and function rehabilitation after replantation, it has high application value.
