Application of intracranial pressure combined with cerebral blood flow monitoring after standard bone flap decompression for patients with severe craniocerebral injury

Abstract:Objective To investigate the effect of intracranial pressure (ICP) combined with cerebral blood flow monitoring on prognosis of patients with severe craniocerebral injury (sTBI) after standard decompressive craniectomy. Methods Ninety-two patients with sTBI admitted to Neurosurgery of Inner Mongolia People′s Hospital from January 2015 to June 2017 were divided into control group and observation group according to the random number stable, 46 cases in each group. Both groups were treated with standard decompressive craniectomy and postoperative routine clinical management. The observation group was guided by ICP combined with cerebral blood flow monitoring after operation. The Glasgow prognosis scale was used to evaluate the therapeutic effect, and the complications of two groups were recorded. Results The total effective rate of the observation group was 67.39%, significantly higher than that of the control group (43.48%), and the difference was statistically significant (P < 0.05). The incidence of electrolyte imbalance and epilepsy of the observation group was significantly lower than that of the control group and the difference was statistically significant(P < 0.05). Conclusion ICP combined with cerebral blood flow monitoring and targeted therapy can optimize cerebral blood flow, control the degree of secondary brain injury, and improve the clinical prognosis of patients with sTBI.
