Abstract:Objective To explore the effect of minimally invasive puncture drainage combined with craniotomy in the treatment of hypertensive cerebral hemorrhage with sudden cerebral hernia and its effect on cerebral blood flow and prognosis. Methods Clinical data of 96 hypertensive intracerebral hemorrhage patients with sudden brain hernia from October 2015 to October 2017 were collected. According to the different operation method, forty-eight cases of minimally invasive puncture and drainage befor the craniotomy evacuation of hematoma and decompressive craniectomy were selected as observation group, and 48 cases of conventional treatment before the craniotomy evacuation of hematoma and decompressive craniectomy as control group. The TCD parameters, curative effect, NHISS score, GOS score and BI value of two groups were observed and compared. Results The total effective rate of the observation group was 91.67%, was higher than that of the control group (75.00%), the difference was statistically significant (P < 0.05). Compared with before operation, there were significant differences in Vm and PI 7 days, 1 month and 3 months after operation of the two groups (P < 0.05), there were significant differences in Vm and PI 7 days, 1 month and 3 months after operation between the two groups (P < 0.05). GOS score of the observation group was better than that of the control group, and BI score 3 months after operation was significantly better than that of the control group, the differences were statistically significant (P < 0.05). Conclusion The curative effect and cerebral blood flow and prognosis of minimally invasive drainage combined with craniotomy for the patients of hypertensive cerebral hemorrhage with sudden cerebral hernia are better than conventional craniotomy. It is an effective method for the patients of hypertensive cerebral hemorrhage with sudden cerebral hernia.
