Comparison on the effect of photodynamic therapy with different concentrations of 5-aminolevulinic acid in the treatment of CIN1/2 patients with HR-HPV infection

Abstract:Objective To analyze the efficacy of photodynamic therapy with different concentrations of 5-aminolevulinic acid (5-ALA) in the treatment of cervical intraepithelial neoplasia (CIN) 1/2 patients with high risk of human papillomavirus (HR-HPV) infection. Methods A total of 80 cases of CIN1/2 patients with HR-HPV infection who treated in Yixing People’s Hospital of Jiangsu Province from January 2017 to June 2019 were selected as the study objects. They were divided into the observation group and the control group according to random number table method, with 40 cases in each group. The control group was given low-concentration 5-ALA, and the observation group was treated with high-concentration 5-ALA. The number of patients with mild cervical abnormalities or normal cervical, HR-HPV negative conversion rate, interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) levels and adverse reactions at 3 and 6 months after treatment were compared between the two groups. Results There were no statistically significant differences between the two groups in the number of mild cervical abnormalities or normal cervical and HR-HPV DNA conversion negative rate of CIN1 patients between the two groups at 3 and 6 months after treatment (P > 0.05); the number of mild cervical abnormalities or normal cervical and HR-HPV DNA conversion negative rate of CIN2 patients in the observation group were higher than those in the control group (P < 0.05). IL-6 level at 3 months after treatment was higher than that before treatment, and TNF-α level was lower than that before treatment; the levels of IL-6 and TNF-α in the observation group were higher than those in the control group (all P < 0.05). None of the patients had secondary infection, bleeding, cervical stenosis or scar formation. Conclusion High concentration 5-ALA photodynamic therapy for CIN1/2 patients with HR-HPV infection has a significant clinical effect, which can effectively improve the negative rate of HR-HPV, and improve the level of inflammatory factors.
