保留乳房的乳腺癌切除术与改良根治术治疗绝经前乳腺癌患者的效果

摘要目的探讨保留乳房的乳腺癌切除术与改良根治术治疗绝经前乳腺癌患者的治疗效果。方法选取2016年5月—2017年5月山东省蓬莱市人民医院就诊的115例绝经前早期乳腺癌患者为研究对象，根据手术方式将其分为观察组（58例）和对照组（57例）。观察组手术方式为保留乳房的乳腺癌切除术，对照组手术方式为改良根治术。比较两组临床效果。比较两组手术时间、术中出血量、引流量、住院时间及生活质量评分。观察两组术后6个月及2年肿瘤复发率和远处转移率。结果观察组总有效率高于对照组，差异有统计学意义（P ＜ 0.05）。观察组手术时间、住院时间短于对照组，引流量少于对照组，差异均有统计学意义（均P < 0.05）；两组术中出血量比较，差异无统计学意义（P > 0.05）。术前两组躯体功能、生理功能评分比较，差异无统计学意义（P > 0.05）；术后，两组躯体功能、生理功能评分均高于术前，且观察组高于对照组，差异均有统计学意义（均P < 0.05）。两组术后6个月及2年肿瘤复发率和远处转移率比较，差异无统计学意义（P ＞ 0.05）。结论保留乳房的乳腺癌切除术治疗绝经前早期乳腺癌患者临床效果显著，可缩短住院时间，提高患者生活质量，值得临床推广应用。

Abstract：Objective To evaluate the effect of breast conserving breast cancer resection and modified radical mastectomy for premenopausal breast cancer. Methods From May 2016 to May 2017, 115 cases with premenopausal early breast cancer who treated in Penglai People′s Hospital were selected as study objects. They were divided into the observation group (58 cases) and the control group (57 cases) according to surgical methods. The clinical effect of the two groups were compared. Operative time, intraoperative blood loss, drainage volume, hospital stay and quality of life score were compared between the two groups. The rate of tumor recurrence and distant metastasis on six months and two years after operation were observed. Results The total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). The operative time and hospital stay of the observation group were shorter than those of the control group, and the drainage volume was less than that of the control group, with statistically significant differences (all P < 0.05). There was no significant difference in intraoperative blood loss between the two groups (P > 0.05). There were no statistically significant differences in scores of physical function and physiological function between the two groups before surgery (P > 0.05). After surgery, the scores of physical function and physiological function in two groups were higher than those before surgery, and the observation group was higher than the control group, with statistically significant differences (all P < 0.05). There were no significant differences between the two groups in tumor recurrence rate and distant metastasis rate on six months and two years after surgery (P > 0.05). Conclusion Breast conserving breast cancer resection for premenopausal early breast cancer patients has a significant clinical effect. It can shorten the hospital stay and improve the quality of life of patients. It is worthy of clinical application.
