关怀型病房的构建对骨巨细胞瘤化疗患者心理特征、睡眠障碍及疼痛信念的影响

摘要目的探讨关怀型病房的构建对骨巨细胞瘤化疗患者心理特征、睡眠障碍及疼痛信念的影响。方法选取2015年5月～2017年5月新疆医科大学第一附属医院收治的确诊并接受化疗的骨巨细胞瘤患者88例，采用随机数字表法将其分为对照组与观察组，每组44例。对照组给予常规护理，观察组在对照组的基础上采用关怀型病房。比较两组患者心理特征、睡眠障碍及疼痛信念。结果与干预前比较，两组患者干预后悲观绝望、恐惧、愤怒、幻想、自私、焦虑不安、孤独及否认患者比例均降低，且观察组显著低于对照组，差异有统计学意义（P < 0.05）。与干预前比较，两组患者干预后睡眠时间、睡眠障碍、入睡时间、睡眠效率、日间功能、安眠药物评分均降低，且观察组显著低于对照组，差异有统计学意义（P < 0.05）。与干预前比较，两组患者干预后对疼痛的控制信念、对疼痛的情绪信念、对疼痛的药物信念、对疼痛的失能信念及对疼痛的耐受信念评分均升高，且观察组显著高于对照组，差异有统计学意义（P < 0.05）。结论将关怀型病房的构建应用于骨巨细胞瘤患者的干预中能够有效缓解患者的不良心理特征，改善睡眠障碍的情况，促进提高患者的疼痛信念。

Abstract：Objective To discuss the influence of the construction of care ward on the psychological characteristics, sleep disorders and pain beliefs of chemotherapy patients with giant cell tumor of bone. Methods From May 2015 to May 2017, in the First Affiliated Hospital of Xinjiang Medical University, 88 chemotherapy patients with giant cell tumor of bone and were selected, they were divided into control group and observation group by random number table method, with 44 cases in each group. The control group was given routine nursing care, the observation group was given the using of care ward on the basis of control group. The psychological characteristics, sleep disorders and pain beliefs of patients in two groups were compared. Results Compared with before intervention, the proportion of patients of pessimistic despair, fear, anger, fantasy, selfishness, anxiety, loneliness, and denial in the two groups after intervention decreased, which of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). Compared with before intervention, the scores of sleep time, sleep disorder, fall-sleep time, sleep efficiency, daytime function, sleeping drug scores in the two groups after intervention decreased, which of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). Compared with before intervention, the scores of the pain control belief, the mood faith of pain, the drug conviction of pain, the pain disability belief and the belief of pain tolerance in the two groups after intervention increased, which of the observation group were higher than those of the control group, the differences were statistically significant (P < 0.05). Conclusion The construction of care ward applied to the intervention of patients with giant cell tumor of bone can effectively alleviate adverse psychological characteristics of patients, improve the condition of sleep disorders, promote the patients′ pain beliefs.
